Merton is part of South West London Health and Care Partnership, which has a commitment to delegate to borough level to ensure local accountability and delivery based on consistency, not uniformity. The partnership published draft health and care plans in March 2019.

Sutton Health and Wellbeing Board (HWB) has been proactive in driving and overseeing extensive developments in integration, prevention and tackling the wider determinants of health, and in holding partners to account. This case study forms part of our integrated care systems (ICS) resource.

Region: North West, West Midlands Theme: Housing (in OPE) The challenge Oswestry is the second biggest market town within Shropshire but suffers from a lack of development from the private sectors due to a number of constraints posed by physical factors such as flood risk and access.

A specialist nursing service for rough sleepers works closely with Bolton Council and local voluntary sector services. It provides support on the streets as well as into hostels and from its town centre base.

Social enterprise Bevan Healthcare provides outreach heath support to rough sleepers from a mobile unit. Together, with its primary care centre and work with hospitals, it is providing vital help to vulnerable people.

The council and its partners in the borough have designed a range of dedicated health services to homeless people, covering everything from GP care and psychological support to dietary advice.

A new centre has been established in Leicester to help those on the streets struggling with substance misuse. Around 150 people are using the facilities each month with many being encouraged into treatment as part of a multi-agency approach.
