Case studies

Sutton Health and Wellbeing Board (HWB) has been proactive in driving and overseeing extensive developments in integration, prevention and tackling the wider determinants of health, and in holding partners to account. This case study forms part of our integrated care systems (ICS) resource.

A stand-alone Recovery Hub has been established as part of Sutton’s substance misuse service. It is situated away from the main treatment clinic giving clients a sense of progress and achievement when they get there, while providing them with the on-going support they need to stay alcohol-free.

The Essex Wellbeing Service offers eight weeks of behavioural support in person, by video, or over the phone to any smoker over the age of 12 who lives in the county.

As soon as COVID-19 hit, a pan-Sussex enforcement cell was set up which included the police, Brighton and Hove City Council, the two county councils and the network of district and borough councils.  

The River Bourne Club is a private members’ health club with a fully-equipped gym and swimming pool. The club has been used by both Surrey County Council and the local NHS to run health clinics.

Managing transfers of care: an example of how a local area is working to implement system change. Between January and October 2012, Surrey County Council worked closely with health partners to introduce reliable seven days per week social care services, including earlier weekday mornings and later evenings. This example of a local initiative forms part of our managing transfers of care resource.

A partnership between Surrey County Council and Woking Borough Council’s Women’s Support Centre is ensuring women who are involved in or at risk of becoming involved in the criminal justice system get the support they need for drug and alcohol problems.  

In East Surrey, adult social care and the CCG are working together to pilot the use of discharge to assess for all continuing healthcare (CHC) assessments. This example of a local initiative forms part of our managing transfers of care resource.

The Surrey Index provides information at various levels of geography: boroughs and districts, ward, Local Community Networks, and Primary Care Networks, super output areas, and also allows a comparison with our statistical neighbours, and includes the most recent data available on a whole host of indicators.

Surrey County Council has set up a specialist outreach team to work with the most hardto-reach drinkers.
