Integrated Care and Support Pioneers

The Integrated Care and Support Pioneers Programme was launched in December 2013. It was set up to test new ways to join up people's care around their needs.

Fourteen localities were chosen to pioneer these approaches, supported by national partners. It is a five-year programme, and is based on the commitments set out in the Integrated Care and Support Collaborative publication Our Shared Commitment, which was published in May 2013.

The programme has produced an annual report, setting out the achievements, challenges and learning from the first year of the programme.
