The concept of integrated care has developed as a response to fragmented delivery of health and social care services in some parts of the current health and care system.

In 2015 Autumn Statement, the government announced a requirement that all local areas integrate health and care services by 2020.

The LGA calls for a radical transformation of services in order to meet the needs of a society with increasingly chronic and complex health needs.

Stepping up to the place: the key to succesful health and care integration

Alongside the Association of Directors of Adult Social Services, NHS Clinical Commissioners and NHS Confederation, the LGA has published Stepping up to the Place, a joint vision for a fully integrated system. It represents the first place based vision for integration signed up to by the whole health and care system.

Integration self assessment tool and workshops

In order to deliver the Stepping up to the Place vision for a fully integrated health and care system, we have designed a self-assessment tool and facilitated sessions to support local health and care system leaders.

Integration resource library

We have developed an integration resource library with definitions, FAQs, tools and resources to support the development of local integrated health and care services.

Better Care Fund

The £5.3bn Better Care Fund was announced by the Government in the June 2013 spending round, to ensure a transformation in integrated health and social care.

Sustainability and transformation plans

An STP – sustainability and transformation plan – is a new planning framework for NHS services intended to be a local blueprint for delivering the ambitions NHS bodies have for a transformed health service.

Integrated Care Pioneer and Vanguard programmes

These programmes are testing new ways to integrate health and care services.
