The journey to integration: Learning from the seven leading localities

This independent report details the experience of seven localities in developing integrated care.

The LGA has long advocated the benefits of integrated, person-centred care as a key vehicle to improve people’s health and wellbeing and experience of care, alongside bringing financial sustainability to the health and care system.

This independent report details the experience of seven localities in developing integrated care. It examines each against a range of factors, including care model, leadership, workforce, payment model and information flow. It concludes that it is possible to have significant impact in terms of improved health outcomes and financial sustainability, and sets out the key lessons for other localities to consider in embarking on integration themselves.
