  1. Was the individual or individual's representative asked what their desired outcomes were?
  2. Did the person or their representative feel that the desired outcomes were achieved?
  3. To what extent was the individual or individual’s representative involved in understanding and responding to acceptable levels of risk?
  4. Did the individual or individual's representative understand why people did what they did to try to keep the individual safe?
  5. Did the individual or individual's representative feel listened to during conversations and meetings with people about helping the individual feel safe?
  6. How happy was the individual or individual's representative with the end result of what people did to try and keep the individual safe?
  7. Did the individual or individual's representative feel that the individual is now safer because of the help from people dealing with the concern(s)?
  8. If the individual is now deceased instead of asking Question 7 ask did the individual's representative feel that the individual was made safer because of the help from people who dealt with the concern(s)?
