Case studies

Borders College is a further education institution in the Scottish Borders, Scotland. It offers a wide range of courses, from an introductory level through to advanced courses at Higher National level. It also provides a large number of industry-based training courses for businesses throughout Scotland.  

The health service and local government have worked together closely across the Bradford and Airedale region to provide telemedicine to care home residents and people in the community.

This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people. This co-produced report was commissioned from the Building the Right Support (BTRS) Advisory Group, as part of the wider action plan developed by the Building the Right Support Delivery Board. It has been supported by Partners in Care and Health.

In late 2017 the Technology Enabled Care (TEC) team began working on a project to improve pathways and access to TEC for those who were being discharged from hospital. The aim was to significantly increase the take-up of lifelines and telecare within a vulnerable cohort that would most benefit from them and ensure that people had access to the councils in house Reablement Enhanced Response Service (ERS). This example of a local initiative forms part of our managing transfers of care resource.

Care technology when considered as part of a holistic support offer means it can be used effectively to support people’s independence and positively impact on their lives.

To provide an equitable hospital discharge experience for residents whilst improving performance associated to delayed transfers, a bespoke Hospital Discharge Service was established in October 2017, with the use of iBCF monies to expand the council’s ‘discharge team’ located at the hospital where the greater number of admissions historically occurred. This example of a local initiative forms part of our managing transfers of care resource.

To test progress towards shared ambitions and the effectiveness of the Health and Wellbing Board (HWB), the Board conducted a comprehensive and well-evidenced review process based on information sharing. This example of how local areas are working to implement overall system change forms part of our care and health improvement digital and information resource.

The end to end process from applying for a sponsor licence to securing a person in post, in the UK took a long time. The process was started in July and the recruits didn’t arrive until January so it’s important to forward plan and allow time for overseas recruitment to happen.

A key priority of the Isle of Wight’s ‘Care Close to Home’ Strategy is to deliver significant improvements to the quality of adult social care registered services. This forms part of our adult social care markets and commissioning resource.

The pandemic brought an urgent need to provide support and assistance to families and individuals struggling with a wide range of issues across Cumbria County Council’s 2,613 square miles. To date, more than 27,000 residents have been supported by the council’s COVID-19 emergency helpline.
