Care technology when considered as part of a holistic support offer means it can be used effectively to support people’s independence and positively impact on their lives.

To provide an equitable hospital discharge experience for residents whilst improving performance associated to delayed transfers, a bespoke Hospital Discharge Service was established in October 2017, with the use of iBCF monies to expand the council’s ‘discharge team’ located at the hospital where the greater number of admissions historically occurred. This example of a local initiative forms part of our managing transfers of care resource.

Working across nine councils and with Samaritans to improve media reporting through hosting a high profile suicide prevention event. Council collaborative comprised of the nine councils across Cheshire and Merseyside: Sefton, Knowsley, St Helens, Liverpool, Wirral, Cheshire East, Cheshire West and Chester, Halton and Warrington councils. This case study was done jointly with the National Suicide Prevention Alliance and forms part of our suicide prevention resource.

Drug dependency Bespoke housing solutions

Charnwood Borough Council set up a dedicated business compliance team at the start of the pandemic to support retail businesses and hospitality venues.

Cherwell District Council are a lead organisation as part of Bicester’s Healthy New Town Programme (BHNT) which aims to use housing growth to develop communities that promote health and wellbeing, prevent illness and keep people independent. This case study shows how district councils have improved the health of their local areas.

A team of youth ambassadors led a project in the Oxfordshire district of Cherwell to get children from its most deprived neighbourhoods active. 

Cheshire West and Chester Council (CWaC) have been providing ongoing support for the pre-planning of Primary Care Network (PCN) sites and the roll-out of all COVID-19 vaccination sites across the borough.

Cheshire West and Chester Health and Wellbeing Board (HWB) is a system-leadership board which meets monthly to monitor progress on integrated health and care developments and to ensure that delayed transfers of care meet national targets. This case study forms part of our integrated care system (ICS) resource.

In Cheshire, two CCGs and the council have worked together to engage the wider local community in self care by recruiting a network of champions and setting up a Self Care Award to entice employers to get involved. This forms part of our resource on self care.
