The health and wellbeing board (HWB) is the senior leadership and oversight body for integration and improving wellbeing in South Tyneside. It has taken forward new ways of working to establish an infrastructure for integration and to tackle health inequalities and the wider determinants of health. This case study forms part of our integrated care systems (ICS) resource.

Planning guidance which restricts the number of hot food takeaways in areas with high childhood obesity is part of South Tyneside Council’s whole systems approach to the problem of excess weight.

Summary Local authorities and third-sector organisations in South Staffordshire have come together to provide housing aids and adaptations to help people live independently at home. Streamlining applications, assessments and provision has resulted in a reduction in costs of 40 per cent and huge cuts in waiting times.

South Norfolk Council has worked with its partners to establish a hospital-based team that helps people when they are discharged. The service is reducing stays in hospital and is now being extended elsewhere in the county. This case study shows how district councils have improved the health of their local areas.

South Gloucestershire agreed a new plan for children’s social care in October 2021 which will see an extra £10m invested over the coming three to five years.

The project context

In developing a healthy new town, South Cambridgeshire has re-considered its approach to developing play schemes and encouraging activity. A more adventurous approach is leading to what is being dubbed “playable landscapes”. This case study shows how district councils have improved the health of their local areas.

The council and local NHS have identified social prescribing as a key priority in terms of encouraging self care. They are working with the Richmond Group of Charities and local partners to spread social prescribing equitably and sustainably across the county. This forms part of our resource on self care.

At the start of the pandemic the council set up a homelessness cell - which led to many rough sleepers being encouraged off the streets and into accommodation.

Six core service partners are working together in Solihull to deliver treatment and support to people with alcohol problems. The Solihull Integrated Substance Misuse Service (SIAS) is led by the local mental health foundation trust in partnership with a range of voluntary sector providers and the council’s social care team.
