The objective of the project was to eliminate the barriers above and to implement the ethos of Home First due to recognising the importance and value of people being more likely to improve, adapt and recover in their own homes. This example of a local initiative forms part of our managing transfers of care resource.

The Dundas Shopping Centre in the heart of Middlesbrough town centre is the Live Well Centre, a one-stop shop for healthy living run by the council’s public health team.

A Big Lottery funded programme aimed at tackling social isolation has been running in Middlesbrough for the past seven years. It has helped to fund a range of different activities across the borough for the over 50s. To help people access these activities, extra support has been put in place to make it easier for people to get out-and-about by working with public transport providers and businesses to create an age-friendly environment.

Mid and South Essex residents who are newly diagnosed with depression can be referred to a ‘Wellbeing Calls’ service which provides additional support in the first few weeks after diagnosis. Delivered by two local voluntary sector partners, the service supported almost 700 people in the year 2022/23.

Merton is part of South West London Health and Care Partnership, which has a commitment to delegate to borough level to ensure local accountability and delivery based on consistency, not uniformity. The partnership published draft health and care plans in March 2019.

Home First is an initiative developed through the partnership of Medway Foundation Trust Hospital, Medway CCG, Medway Council and Medway Community Healthcare. It provides support for patients medically fit to be discharged, but who still require additional home support. This example of a local initiative forms part of our managing transfers of care resource.

Working together with the Samaritan’s Media Advisory Service to monitor local media reporting of suicides and provide training local media organisations to develop postvention reporting as a means of prevention. This case study was done jointly with the National Suicide Prevention Alliance and forms part of our suicide prevention resource.

The Derby and Derbyshire Autism Partnership Board (APB) has been established for 10 years, has an autistic co-chair and good representation from commissioners, health, providers, parents, and Experts by Experience (EbE’s). Derbyshire County Council support the APB with Derbyshire Autism Service providing an autism friendly meeting room when required

This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people. This co-produced report was commissioned from the Building the Right Support Advisory Group, as part of the wider action plan developed by the Building the Right Support Delivery Board. It has been supported by Partners in Care and Health.

The ASSIST– advocacy, sustainment, supporting independence and safeguarding team – scheme brings together teams from the district council, Nottinghamshire County Council, Sherwood Forest Hospitals NHS Foundation Trust, Mansfield and Ashfield Clinical Commissioning Group, and Nottingham Trent University. This example of how local areas are working to implement overall system change forms part of our managing transfers of care resource.
