These case studies show how local systems are putting the High Impact Change Model into practice to improve intermediate care, strengthen partnership working and support people to recover, regain independence and return home safely.

HICM: Intermediate Care: Croydon

Overview

In 2017, Croydon Council, the local NHS Commissioning body, local health providers and voluntary organisations formed the One Croydon Alliance. They signed a legally binding 10 year agreement to work as one system, with shared goals and shared responsibility for local people’s wellbeing. The Alliance began by focusing on residents aged 65+, aiming to reduce avoidable hospital admissions, and help older people remain independent for as long as possible.

Case Study

Lancashire: I’ve seen LGA support from both sides

  • Councillor Daniel Matchett has been an LGA peer as well as benefitting from support given to Lancashire County Council
  • He was part of the team reviewing Kent’s Health and Wellbeing Board, which he described as a rewarding experience
  • The LGA also helped him overhaul Lancashire’s Health and Wellbeing Board, which he chairs

Daniel Matchett became a councillor in May 2025, but was immediately appointed Cabinet Member for Health and Wellbeing as Reform UK took control of Lancashire County Council for the first time.
