HICM: Intermediate Care: Croydon

Overview

In 2017, Croydon Council, the local NHS Commissioning body, local health providers and voluntary organisations formed the One Croydon Alliance. They signed a legally binding 10 year agreement to work as one system, with shared goals and shared responsibility for local people’s wellbeing. The Alliance began by focusing on residents aged 65+, aiming to reduce avoidable hospital admissions, and help older people remain independent for as long as possible.

HICM: Intermediate Care: Halton

Overview

Prior to the COVID-19 pandemic, Halton’s intermediate care provision was delivered through several separate services. These included the Rapid Access Rehabilitation Service, a Capacity and Demand Team, falls prevention team, hospital discharge teams and a Frailty team. While each service provided valuable support, they operated largely independently.

Overview

Demand pressures and increasing costs were putting strain on the system, making it difficult to maintain service quality and meet the needs of service users efficiently. There was recognition that no single partner could deliver systemic improvement alone; effective collaboration across Stoke-on-Trent City Council, Staffordshire County Council and Midlands Partnership Foundation was essential for meaningful change.

Gloucester City Council - options to acquire and develop properties

Working collaboratively with the council, we designed and executed an options appraisal to support Gloucester City Council to identify and evaluate the optimum delivery strategy.

Gloucester City Council are now pursuing a strategy to identify potential joint venture partners with whom they will seek to acquire a range of properties meeting differing needs, including temporary accommodation.

A report was commissioned by Havering’s Joint Commissioning Unit to understand the current needs and pressures across children and adults social care services and estimate the future accommodation needs for looked after children, care leavers, and children and adults with disabilities.

Calderdale Borough Council - Prevention strategy for 16/17 year old homeless presentations

The numbers of 16/17 yr. olds requiring emergency homeless accommodation has increased over a number of years. Despite having developed planned supported accommodation pathways for young people in Calderdale, the need for emergency accommodation is still high. In line with HRA and the need to reduce this number we need to consider how we are able to prevent young people, families other agencies in looking towards homeless accommodation as the remedy for a myriad of other related issues.
