The learning from the programme has increased the evidence base about the benefits of prevention, early intervention and the integration of services – all fundamental to reforming the care and support system. Two-thirds of the 146 projects were focused on communities while the remaining centred on hospitals. A further 530 small upstream projects were commissioned from the third sector.
