Wiltshire's Help to Live at Home service is an outcomes-based model for commissioning domiciliary care with payment-by-results on rehabilitation. Since its launch, it has enabled the council to place fewer people every year in long-term care and to make significant financial savings.
Payments to providers are based on whether the outcome has been achieved. This has resulted in £11.6million worth of savings delivered over the duration of the project and the council is looking to extend the service to other customer groups. It estimates that this reduction in demand is worth around £5 million since launch.
