Implementing the high impact change model
The support offers expert advice and guidance to systems on using the High Impact Change Model to self-assess local strengths, identify areas for development and implement changes. The offer includes national and regional workshops and peer/adviser bespoke support as well as shared learning and case studies.
Support is jointly delivered by the Better Care Support Team, the Emergency Care Improvement Programme hosted by NHS Improvement and the Local Government Association’s Care and Health Improvement Programme.
