Local area approaches: case studies

Eastern region

Local area partners from across the Eastern region have worked together to develop a Market Relations protocol. The protocol – developed with people, carers, advocates, commissioners and providers – is about committing to working together across the region to ensure the availability of high-quality person-centred support for people.

Barnsley

Key things that the case study covers:

  • fundamental reform of learning disability services using the ‘progression model'
  • significant efficiency savings for the local authority and CCG
  • focus on enabling people and giving people choice and control
  • importance of specialist social work and case management expertise
  • establishing a culture that strikes the right balance between safety and autonomy
  • looking at increasing specialist crisis support in the community to prevent crisis hospital admission – in partnership with other authorities within region.

Dudley

Key things that the case study covers:

  • joint commissioning arrangements including joint commissioning post
  • integrated community learning disabilities team across social care and health and continued oversight of hospital admissions by the community team

Gloucestershire

Sunderland

Key things that the case study covers:

  • long-standing pooled budget for people with learning disabilities with high level needs and established links between commissioning and care management 
  • regular contact with NHS specialised commissioning
  • working together with people with learning disabilities and families through a Transforming Care Project Board which reports into the Health and Wellbeing Board
  • community treatment team providing outreach and crisis support to maintain people in the community
  • multi-agency children resolution group which agrees the commissioning of proposed new placements and reviews any issues with current placements.

Islington

Key things that the case study covers:

  • Positive Behaviour Support approach to form part of legal contracts with providers
  • ‘Circles of Protection' service for people placed out of borough
  • pooled funding agreement between health and social care providing a ‘one stop' service for people
  • a pod' model for accommodation to tackle the shortage of local extra care supported housing, and limited building development space
  • invest to save funding to develop local transition services for younger adults
  • joint working with neighbouring boroughs to enable people to live closer to home, irrespective of borough boundaries

Bedford

Key things that the case study covers:

  • Progression pathway model for strategic and operational planning
  • Starting the planning process for young people's future needs at least 5 years prior to transition from children's to adults' services
  • Putting effort into understanding the best communication methods, both for providing general information on services and for communicating with individuals and their families
  • In-house provision uses a pre-screening tool to assess values and attitudes of potential staff prior to formal interview

Leicester, Leicestershire and Rutland

Key things that the case study covers:

  • Partnership working across three councils and CCGs
  • NHS England specialised commissioning works closely with local commissioners

  • Focus on early intervention with young people and families to prevent or reduce the need for future services
