This report calls for a radical transformation of services in order to needs of a society with increasingly chronic and complex health needs. The vision paves the way for integration and transformation to happen faster and to go further so that integrated and person-centred care becomes the mainstream.
The LGA has released a report with the NHS Confederation, the Association of Directors of Adult Social Services and NHS Clinical Commissioners that is the first place based vision for integration signed up to by the whole health and care system.
