HICM: Intermediate Care: Hertfordshire

Overview

Hertfordshire County Council had previously sought to embed a reablement ethos in care provision by placing occupational therapists (OTs) within generic teams, largely managed by social work qualified managers. This approach had delivered limited progress, and the expected benefits did not fully materialise. The Council therefore decided to take a new approach, creating a dedicated Enablement Team.

HICM: Intermediate Care: Somerset

Overview

In Somerset, system leaders worked together on a shared ambition to redesign intermediate care. An Intermediate Care Board was established to steer the work. Whilst in its infancy, this Board was co-chaired by the DASS and an NHS COO. A transformation partner was commissioned to develop a joint data set, which had been identified as a key area of opportunity.
