HICM: Intermediate Care: West Sussex

Colleagues in West Sussex were experiencing high demand for long-term adult social care services, needs becoming more complex, and continued financial pressures. Action was taken to strengthen reablement as a core intervention, promoting independence and managing long term care demand. The Community Reablement Service was recommissioned, with approximately £0.72m additional investment.

HICM: Intermediate Care: Croydon

In 2017, Croydon Council, the local NHS Commissioning body, local health providers and voluntary organisations formed the One Croydon Alliance. They signed a legally binding 10 year agreement to work as one system, with shared goals and shared responsibility for local people’s wellbeing.

HICM: Intermediate Care: Halton

Prior to the COVID-19 pandemic, Halton’s intermediate care provision was delivered through several separate services. These included the Rapid Access Rehabilitation Service, a Capacity and Demand Team, falls prevention team, hospital discharge teams and a Frailty team. While each service provided valuable support, they operated largely independently.

HICM: Intermediate Care: Stoke and Staffordshire

Demand pressures and increasing costs were putting strain on the system, making it difficult to maintain service quality and meet the needs of service users efficiently. There was recognition that no single partner could deliver systemic improvement alone; effective collaboration across Stoke-on-Trent City Council, Staffordshire County Council and Midlands Partnership Foundation was essential for meaningful change.

Analysis of Safeguarding Adults Reviews targeted briefings

The Care and Health Improvement Programme (CHIP) safeguarding workstream commissioned Suzy Braye, Michael Preston-Shoot and Research in Practice to undertake a review of SARs published in 2018/18 and 2918/19 to inform future priorities for sector led improvement in safeguarding adults’ practice. This is a ‘once in a decade’ opportunity to identify learning from SARs at a national level, as there is no regular reporting mechanism.
