East Riding is an area within Humber Coast and Vale Sustainability and Transformation Partnership, which is establishing several integrated care partnerships (ICPs). This case study forms part of our integrated care systems (ICS) resource.
East Riding Health and Wellbeing Board (HWB) provides high-level strategic direction and oversight for a place-based approach. It is a mature forum, with mutual trust and a culture of support and challenge. It provides strong democratic input into health and care leadership at place level and uses a systems approach to collaborate on complex issues.
The strategic vision of the HWB is set out in the joint health and wellbeing strategy, which takes a life-course approach with two core themes of prevention and personal and community resilience. East Riding Place Partnership, which involves 11 key health and social care related organisations, including blue light services, delivers a work programme for how partners work together as a single system.
For example, partners are developing ‘collective accountability and responsibility’ for population health and care outcomes in which key strategies, such as dementia and mental health, healthy weight and carers, are delivered as a single system.
“I would very much hope to see health and wellbeing boards (HWBs) continue to represent ‘place’ at the right level to integrate into the wider geography of integrated care systems (ICSs). Better communication between boards could bring together the full picture to underpin the ICS priorities. There is now an opportunity to move forward from lessons learnt, and for HWBs and ICSs to make a unique contribution around delivering change at the right scale.”

The clinical commissioning group governing body and the council have recently committed to establishing a joint committee, responsible for providing a focus on:
- alignment and development of joint strategic plans and priorities
- alignment and delivery of joint commissioning plans and functions
- alignment of spending priorities, budgets and risk-sharing arrangements
- influence at ICS level.
The HWB promotes asset-based working. ‘Active Withernsea’ is a whole-system project overseen by the HWB, aimed at improving physical and mental health in an area with poor health outcomes and high demand for services. This project has had many positive outcomes:
- multi-million-pound investment from Sport England
- development and implementation of a local social prescribing model
- savings related to reducing demand on hospital services
- leisure centres seen as wellbeing centres.
Learning from the project is being used to support the development of the primary care networks, community hubs and social prescribing, which are HWB priorities.

East Riding is part of a proposed ICP with Hull to work at scale across community and primary care, planned care, and urgent and emergency care. Three initial projects have been identified: community paediatrics, frailty and ambulance services. The work of the ICP will be overseen by the HWB. The majority of health and care planning and delivery will happen at place level, and the ICS and ICPs will work on issues best tackled at greater scale.
