Rotherham is part of South Yorkshire and Bassetlaw, a first-wave integrated care system (ICS). The places within the ICS are developing as integrated care partnerships (ICPs), and a key message from the ICS is that ‘place’ is put first.
West Yorkshire and Harrogate Health and Care Partnership (HCP) covers 2.6 million people. It is made up of around 50 local health and care networks, eight local authority areas, seven local care partnerships and six place plans (Bradford District and Craven, Calderdale, Harrogate, Kirklees, Leeds and Wakefield). This case study forms part of our integrated care systems (ICS) resource.
Merton is part of South West London Health and Care Partnership, which has a commitment to delegate to borough level to ensure local accountability and delivery based on consistency, not uniformity. The partnership published draft health and care plans in March 2019.
A nurse prescriber from the drug and alcohol service has started doing outreach work with the homeless support team. The work has focused on one particular town centre in the district after concerns were raised about a core group of rough sleepers who were not engaged with treatment.
The council and its partners in the borough have designed a range of dedicated health services to homeless people, covering everything from GP care and psychological support to dietary advice.
A new centre has been established in Leicester to help those on the streets struggling with substance misuse. Around 150 people are using the facilities each month with many being encouraged into treatment as part of a multi-agency approach.
Support is provided to homeless people while they are in hospital. It ensures when they are ready to be discharged they will not return to the streets. Over the past two years no-one has been released without accommodation to go into.
Work Local is the Local Government Association (LGA)’s positive vision for an integrated and devolved employment and skills service – bringing together information, advice and guidance alongside the delivery of employment, skills, apprenticeships and wider support for individuals and employers. This case study highlights how Portsmouth City Council and Southampton City Council worked together to implement a similar approach.
