In late 2017 the Technology Enabled Care (TEC) team began working on a project to improve pathways and access to TEC for those who were being discharged from hospital. The aim was to significantly increase the take-up of lifelines and telecare within a vulnerable cohort that would most benefit from them and ensure that people had access to the councils in house Reablement Enhanced Response Service (ERS). This example of a local initiative forms part of our managing transfers of care resource.

Active Herts is a research led and independently evaluated project which uses evidence-based behaviour change techniques to support inactive adults to increase their physical activity levels.

Public health in Portsmouth is threatened by poor air quality. Across the UK, it is estimated that air pollution contributes to between 28,000 and 36,000 deaths per year.

With the help of Mind, Oxford City Council set about improving rates of sickness absence related to stress and anxiety by better equipping line mangers and staff to understand and talk about mental health issues at work.

The story so far Gareth Spooner is employed as staff wellbeing coordinator at Portsmouth City Council in a post that was initially funded jointly by HR and Public Health and has since become part of the HR establishment.

Norfolk CC implemented a staff engagement survey with an underpinning research base, to understand the unique selling point for working in the public sector. 

The Productivity Experts Programme grant funding supported the redesigning of the commissioning of our placement function for Children’s Services and considering future integration with the Adult’s services; as well as developing effective performance management processes and systems, and a quality assurance framework.

Leeds has integrated its substance misuse services – and this is helping boost innovation. Support is being moved into GP centres, there is an enhanced family service and a new team to work with those with earlystage alcohol problems.

Six core service partners are working together in Solihull to deliver treatment and support to people with alcohol problems. The Solihull Integrated Substance Misuse Service (SIAS) is led by the local mental health foundation trust in partnership with a range of voluntary sector providers and the council’s social care team.

Staff from Gateshead’s substance misuse service have a presence at the borough’s main hospital four days a week. This gives them a chance to identify and engage patients who have alcohol-related problems.
