Wakefield Connecting Care programme, which incorporates the BCF, is a partnership between the CCG and the council, working closely with NHS providers and the voluntary and community sectors.  A key element of the programme is establishing integrated care hubs – care closer to people’s homes through integrated teams based around GP practices.

London Borough of Richmond upon Thames and the CCG are moving jointly towards an integrated outcomes-based adult community health and social care service. This aspiration now also includes mental health services and early thinking has started for children’s services

Somerset County Council and Somerset CCG have been developing joined up care for several years, and this has accelerated through initiatives such as joint commissioning, an integrated data system, and the BCF programme.  A model for ambitious, whole-system reform is now being developed

How public health became involved When based in the PCT, public health was central to both NHS strategic planning and joint planning with the council. This role was maintained when it transferred to the council, with public health taking the lead for several aspects of the BCF and wider integration.

Ground-breaking data analysis is enabling Cheshire Fire and Rescue Service (CFRS) to pinpoint precisely which residents are most at risk of fire and falls and in need of intervention.

The tragedy of 13 separate fire deaths in one year has led to a transformation in how Dorset firefighters work with partner agencies to enable residents to live safely in their own homes.

These are suggestions of how to measure the contribution of culture and sport to better outcomes for children and young people.

Supporting the council to create an economic development action plan which addressed the key issues and challenges and engaged all stakeholders in delivery and support activity and creating a new perception of Dengie as a place to grow businesses.

The borough identified the need to work with at risk communities to increase reporting for support (voluntary or statutory support) and the need for greater understanding among professionals in supporting FGM victims.

Councillor Gingell, Cabinet Member for Health and Adult Services, raised the issue of FGM at a full council meeting in December 2013. A Full Council Motion (Appendix A) was introduced to condemn the practice, which was unanimously supported.
