This case study describes Nottingham’s progress developing integrated care arrangements. It is part of an evidence review commissioned from the Institute of Public Care.

Havering Council has developed an online portal to support two-way information sharing between providers and the council.

To improve the relationship between the council and external adult social care providers, Havering Council recognised a need to develop a two-way communications model between all providers and the council. The model would support a more equal relationship, focusing on the best possible outcomes for vulnerable residents. This forms part of our adult social care markets and commissioning resource.

In the last year, there has been a major transformation in how partners Ipswich and East Suffolk work together to tackle delayed transfers of care (DTOCs). For example, on 1 February 2018, 10 DTOCs were reported; in the same period in 2017 it was 49. Bradford District and Craven has a population of around 590,000 people. It is divided into two health and care partnership areas with distinct geographies and demographics. A high priority for public health on transferring to local government in Dorset and even more so since the advent of STPs has been to make health improvement provision more consistent and equitable across the county and to join up support for different lifestyle risks. It was also a priority to draw on proven theories of behaviour change and to monitor outcomes. LiveWell Dorset is an integrated single service delivered by one contractor to support change across a range of risks and behaviours. 

Joint contracting arrangements – for care homes, community based and voluntary sector provision – are headed up by the director of integrated commissioning, with lead commissioners incorporating children and maternity, disabilities and older people. The Royal Borough’s ambition, and challenge, is to continue to deliver effective and sustainable adult social care services, which meet residents’ needs, against a backdrop of diminishing financial resources.
