Care homes COVID-19 crisis prompted a local innovative, evidence-based approach to tackling COVID-19 infections/outbreaks and other challenges faced by Sutton care homes.

Prior to the move into the discharge hub, work had already taken place to replace the ward discharge Officers with a more administrative role, new staff were recruited at band 4 on each ward within the Trust. A decision was also made that each directorate would have a supervisor Band 5 post to over-see the work of the discharge officers. Support is provided to homeless people while they are in hospital. It ensures when they are ready to be discharged they will not return to the streets. Over the past two years no-one has been released without accommodation to go into.
