In the Pan-Lancashire flexible agreement, 39 providers have been selected through competitive tendering to provide supported living to people with a learning disability and/or autism who display behaviour that challenges. Norfolk County Council has established a high-performing specialist county team that supports the emotional, cultural, health, social and educational needs of Unaccompanied Asylum-Seeking Children (UASC) who arrive in the county.

West Sussex County Council works with a range of providers to deliver the NHS Health Check programme across the county. This includes offering checks in community settings such as libraries, village halls, workplaces and high-street hubs, as well as in clinical settings.

Support is provided to homeless people while they are in hospital. It ensures when they are ready to be discharged they will not return to the streets. Over the past two years no-one has been released without accommodation to go into.

Celebrating the area’s rich history of innovation, the Museum of Making in the Derwent Valley Mills UNESCO World Heritage Site opened in May 2021 as a contemporary space telling Derby’s 300-year history of making.

Prior to the move into the discharge hub, work had already taken place to replace the ward discharge Officers with a more administrative role, new staff were recruited at band 4 on each ward within the Trust. A decision was also made that each directorate would have a supervisor Band 5 post to over-see the work of the discharge officers.
