Managing transfers of care: an example of how a local area is working to implement system change. The model in Hull, which forms part of a system- wide approach to reducing delayed transfers of care, is based on the principle of ‘active recovery’.

Huntingdonshire District Council has worked with its local academic health science network to encourage cancer and cardiac rehab patients to become active and involved in the self-management of their recovery. Patients are being taught how to use remote monitoring technology, while taking part in exercise classes. This forms part of our resource on self care.

This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people. This co-produced report was commissioned from the Building the Right Support (BTRS) Advisory Group, as part of the wider action plan developed by the Building the Right Support Delivery Board. In January 2022, the uptake rate of the Healthy Start scheme in Hounslow was 49 per cent After implementing a series of awareness raising and educational interventions, the uptake was 57% in January 2023. Following another year of more targeted promotion of the scheme, the uptake was 71 per cent in 2024.
