Bradford District and Craven has a population of around 590,000 people. It is divided into two health and care partnership areas with distinct geographies and demographics. This example of how local areas are working to implement overall system change forms part of our managing transfers of care resource.

In the last year, there has been a major transformation in how partners Ipswich and East Suffolk work together to tackle delayed transfers of care (DTOCs). For example, on 1 February 2018, 10 DTOCs were reported; in the same period in 2017 it was 49. Funding was provided for a series of engagement events to raise awareness of cobot (collaborative robot) technology and the potential benefits it can offer the UK care sector.

To improve the relationship between the council and external adult social care providers, Havering Council recognised a need to develop a two-way communications model between all providers and the council. The model would support a more equal relationship, focusing on the best possible outcomes for vulnerable residents. This forms part of our adult social care markets and commissioning resource.

Following the development of the wellbeing offer for NHS staff affected by COVID-19 in April 2020, the NHSE & I regional team in the south east of England felt that there needed to be a similar resource for social care staff.

82-year-old Oxfordshire-based Jennifer Baird was diagnosed with dementia after a stroke. She became confused and struggled with practical tasks. A sharer, Judith O’Boyle, was found through Share & Care Homeshare, and she lived with Jennifer for 12 months.

United Pride Friends is a group service for members of the LGBTQ+ community with Autism and Learning difficulties. The primary purpose of their group is giving members a safe place to meet new people and make friends. This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people.

There is strong partnership working in Greenwich to develop and improve the local offer. The overall objective is that people at the end of their lives and their families can be well supported and their outcomes improved through care and support that is integrated around the individual. The development of a transitions service offered by Hammersmith & Fulham Council demonstrates the good practice set out in several key domains of this outcomes and improvement framework.
