Homerton University Hospital’s movement towards seven-day working to improve patient flow has been a long journey that has taken a few of years and gone through a number of iterations in terms of changes to systems and processes. The Fylde Coast Health and Social Care Career Academy is a result of the partnership between local care providers, Blackpool and The Fylde College, Blackpool Teaching Hospital NHS Foundation Trust and Blackpool Council.

A high priority for public health on transferring to local government in Dorset and even more so since the advent of STPs has been to make health improvement provision more consistent and equitable across the county and to join up support for different lifestyle risks. It was also a priority to draw on proven theories of behaviour change and to monitor outcomes. LiveWell Dorset is an integrated single service delivered by one contractor to support change across a range of risks and behaviours. 

The aims of the SCDIA Digital Discharge to Assess project were to develop a cloud-based case management system that did not depend on a single IT partner or supplier; allow users (with appropriate permissions) to capture, update, track, and report on data about a person’s journey through the Discharge to Assess process; create a ‘single version of the truth’, with a raft of associated patient benefits and service efficiencies.
